| From Date | Type the beginning search date in the MMDDYYYY format. Or, select a date from the calendar. |
| To Date | field Type the ending search date in the MMDDYYYY format. Or, select a date from the calendar. |
❏ Click Retrieve. A list of data that matches the search criteria is displayed.
❏ Select an amendment number from the list. Otherwise, click Cancel.
