| Measurable Secondary Goals |
Select whether or not there were appropriate measurable postsecondary goals in the areas of training, education, employment, and, where appropriate, independent living skills in a student’s IEP. |
| Updated Secondary Goals |
Select whether or not the postsecondary goals set in the IEP were updated annually. |
| Transition Assessment |
Select whether or not there is evidence that the measurable postsecondary goals were based on age-appropriate transition assessments. |
| IEP Transition Services |
Select whether or not the student's IEP includes transition services that will reasonably enable the student to meet their postsecondary goals. |
| Courses Study Secondary Goals |
Select whether or not the transition services included courses of study that will reasonably enable the student to meet their postsecondary goals. |
| Transition Service Needs |
Select whether or not there is/are annual IEP goals related to the student’s transition service needs. |
| ARD Invited Student |
Select whether or not there is documented evidence the student was invited to the Admission, Review, and Dismissal (ARD) Committee meeting where transition services were discussed. |
| ARD Invited Representative |
Indicate whether or not there is evidence that a representative of any participating agency was invited to the Admission, Review, and Dismissal (ARD) Committee meeting with the prior consent of the parent or student who has reached age of majority. |
