From Date (MMDDYYYY) Blank = first day of school.)

Select the date for the first day of school.

To Date (MMDDYYYY) Blank = last day of school.)

Select the date for the last day of school.

Page Break on Grade Level (Y, N, Blank=Y))

Type the two-character grade level, click ellipsis button to select the grade level, or leave blank to select all grade levels.
