SPED Video Camera Request

This page allows you to make a request for a camera placement in a special education setting.

❏ Select the three-digit Campus ID and click Retrieve.

Existing campus requests are listed.

❏ Click +Add.

A blank row is added to the grid.

Request ID	Select to indicate the unique identifier for the individual special education classroom video camera request.
Camera Request Received Date	Type the date the local education agency received the special education classroom video camera request using MMDDYYYY format. Or, click to select the date from a calendar.
Camera Requestor	Select the category to which the person belongs who is making the request for a video camera to be installed in a special education classroom.
Camera Status	Select if the special education camera request has been authorized, denied, or withdrawn.
Camera Status Determination Date	Indicates the date the special education camera request was authorized, denied, or withdrawn using MMDDYYYY format. Or, click to select the date from a calendar.
Denial Reason	(if applicable) Select the reason the camera request was denied.
Camera Operational Date	Type the date camera became operational using MMDDYYYY format. Or, click to select the date from a calendar.

	Delete a row. Click to delete the selected row. The row is shaded red to indicate that it will be deleted when the record is saved. Click Save. A message is displayed confirming that you want to delete the row. Click OK to delete the row. Click Cancel not to delete the row.
