❏ Select the three-digit Campus ID and click Retrieve.

Existing campus requests are listed.

❏ Click +Add.

A blank row is added to the grid.

Camera Request Received Date	Type the date the camera was requested using MMDDYYYY format. Or, click to select the date from a calendar.
Camera Requestor	Select who requested the camera.
Camera Status	Select the status of the camera request.
Camera Status Determination Date	Type the date the camera XXXX using MMDDYYYY format. Or, click to select the date from a calendar.
Denial Reason	(if applicable) Select the reason camera usage was denied.
Camera Operational Date	Type the date camera usage began using MMDDYYYY format. Or, click to select the date from a calendar.

	Delete a row. Click to delete the selected row. The row is shaded red to indicate that it will be deleted when the record is saved. Click Save. A message is displayed confirming that you want to delete the row. Click OK to delete the row. Click Cancel not to delete the row.

❏ Click Save.
