| For Static Forms: |

Type the date on which the parent manually submitted the form in the MMDDYYYY format, or click  to select the date from a calendar, and click Save. |
| For Dynamic Forms: |
| The most recent date on which the parent submitted a change via txConnect is displayed. |
| For Standard Forms: |
| The most recent date on which the parent submitted or acknowledged the form via txConnect is displayed. |
