SHS0102 - Immunization Due Report

This report lists immunizations due as of a specified date. It can be printed for all immunizations or for a specific type.

Grace periods for immunizations

Run the report:

As of Date (MMDDYYYY)

Type the date, or click to select the date from a calendar.

Leave blank to use the current date.

If Y, the As of Date must be the first day of school for the upcoming school year. If this date is not yet known, use an estimated first day of school (e.g., 09/01).

Immunization Type (Blank for All)

Type the two-digit immunization type code, or click to select the immunization. Leave blank to select all immunization types.

Campus ID (Blank for All)

Type the three-digit campus ID, or click to select the campus. Leave blank to select all campuses in the district.

NOTE: If you select to create the report for all campuses, any campuses excluded from district reporting (i.e., campuses that have the Exclude from District Reporting field selected on Registration > Maintenance > Campus Profile > Campus Information Maintenance > Control Info) are not included in the report. However, you can generate the report for an individual excluded campus by entering the campus ID.

Active Code (1=Active, 2=Inactive, Blank for All)

1 - Select active students only.

2 - Select inactive students only.

Blank - Select all students.

Grade Level (Blank for All)

Type the two-character grade level, click ellipsis button to select the grade level, or leave blank to select all grade levels.

Student ID (Blank for All)

Type the six-digit student ID number, including all leading zeros. Or, click to select the student. Leave blank to select all students.

Use Next Year Grade Level (Y, N)

Y - Use the student's next year grade level.

If Y, the As of Date must be the first day of school for the upcoming school year. If this date is not yet known, use an estimated first day of school (e.g., 09/01).



N or blank - Do not use the student's next year grade level.

For students in PK who will still be four years old on September 1 of the next school year, the grade level will not be incremented to KG; therefore, KG rules will not apply to the student (DTP, Polio, MMR, Varicella, and Hep A).
